PLEASE NOTE:
By participating in activities in a program at Basketball R Us, I, the undersigned participant, acknowledge and agree to the following: Assumption of Risk:
- Basketball R Us activities, running, scrimmaging, training, and other attractions, have inherent risks and dangers.
- These risks include, but are not limited to, falls, collisions, sprains, fractures, paralysis, and even death.
- All risks associated with participation in these activities are knowingly and voluntarily assumed.
Release of Liability:
- Basketball R Us and its affiliates, owners, employees, instructors, schools, coaches, officials and/or agents (collectively, ” Basketball R Us”) are released and discharged from any and all claims, demands, actions, or causes of action arising out of or related to participation in these activities. This includes injuries or damages sustained as a result of negligence on the part of Basketball R Us.
- The right to bring a lawsuit against Basketball R Us for any injuries or damages sustained during the visit is waived.
Acknowledgment of Rules and Safety Guidelines:
- All Basketball R Us rules, procedures, and safety guidelines must be followed, along with all instructions from Basketball R Us team members.
- Failure to comply with these rules may result in the loss of jump time or expulsion from any Basketball R Us program.
Medical Insurance:
- Adequate insurance to cover any injury or damage that may be caused or suffered while participating is certified, or the costs of such injury or damage will be borne by the participant.
Medical Release:
- Consent is given to receive medical treatment in the event of injury, and the participant is responsible for all costs associated with such treatment.
Parental Consent for Minors:
- If the participant is under 18 years of age, the parent or legal guardian acknowledges reading and understanding this disclaimer and consents to the minor’s participation in Basketball R Us activities.
- Full responsibility for the minor’s safety and well-being while at Basketball R Us is assumed by the parent and/or legal guardian.
APPROVAL
By signing up on this form, I/we give my/our permission to Basketball R Us to use photographs and videos of my/our child for publicity in order to increase community awareness of Basketball R Us programs in any and other media without limitation.
